Who Can Apply...

In order to apply and  receive a United States Passport
you must follow instructions as  outlined  below.
To make the  process easier to complete we suggest 
you Print This Page and  follow the steps in the order
presented.   Please call with any questions. 215.739.3844

Before calling or doing anything, read thru all 10 steps then go back to step 1 and begin following each step as instructed.  THANK YOU

1.   Authorization Letter- Allows us to act as an agent on your behalf.

• Please print and complete 2 copies of the Authorization Letter(click)

2.   Proof of Citizenship - You must provide ONE of the following.

• Previous U.S.. Passport more than 15 years old.
• Certified Birth Certificate issued by state, county or city (if born in USA)
• Certificate of Citizenship or FS-240, DS-1350 (if born outside the USA)
• Certificate of Naturalization

3.   Proof of Identity

• Valid drivers license with photograph issued more than 6 months ago.
• If you do not have a drivers license or a drivers license that meets the
  requirements above then you must supply Personal Documents.

4.  Evidence of Immediate Travel - supply 2 copies of 1 of the following

• Copy of round trip ticket indicating travel to a foreign country.
• Travel itinerary stating reserved or confirmed.
• Business travelers may submit a Business Letter of Expedite, if the
  above is not available. Call for clarification.

5.  Official Form DS-11 - Complete, sign in agents presence.

• Online DS-11 Application (fill in empty fields then print)
• DS-11 PDF File   (Print blank form and fill in neatly by hand)      

6.   Passport Photographs - taken by a professional. (Guidelines)

• Obtain 2 Passport photos. (Major Pharmacies now do this. CVS, Walgreen's etc.)

7.     Fees - Two(2) separate payments.

• 1st Check - Government fees: Check or MO payable to ‘US Dept. State’,
                   $135 adults, $120 children
• 2nd Check - Payable to ‘PhiladelphiaPassports.com' (Fees on Order Form)

8.     Order Form- PhiladelphiaPassports.com,Inc. exclusive Order Form

• Print our Order Form, complete and be sure to include with order.

9.     Authorization Process -  Click HERE to find your nearest local agent.

• An Acceptance Agent will verify your identity and witness your signature.
• Child passports require both parents and the child to be present, or a
  signed, notarized Parental Consent Letter from the non-present parent.

Before appearing in front of an Acceptance Agent, please be sure
you have all the required documents.
This is what you will need...

Acceptance Agentsare found in local Post Offices and Court Houses. (not every
Post Office or Court House is an Acceptance Agent) Most acceptance facilities are
open 9:00am - 4:00pm. We urge you to call your local Acceptance Agent to verify
their hours of operation. Click Here to search by zip code for the nearest agent.

Do NOT sign the DS-11 until instructed to do so by the Acceptance Agent. You
will sign the application after the Acceptance Agent verifies your identity and
approves your application.

Tell the Acceptance Agentthat a ‘ courier service will hand carry the materials’
to the Passport Agency. This Department of State policy is called
‘Hand carrying of Executed Documents’. The policy is detailed on
pages 31 and 44 of the Acceptance Agent Manual.

The Acceptance Agent will check your proof of identity, endorse the documents
and seal them in an envelope marked ‘To be opened by authorized passport
officials only’. Do NOT open the envelope after it is sealed.

Some Acceptance Agents will seal one of our Authorization Letters in the envelope.
In this case, you MUST also attach one copy with an original signature to the envelope.

Print your name, birth date, departure and date on the outside of the sealed envelope.

10.    FedEx Shipping Instructions - Ship to address on top of Order Form.    
            (Your package must contain the items below, clipped or stapled together)

You must choose the service "Priority Overnight", next day delivery by 10:30am. 
(UPS offers a similar service)

Call us. We must know the package is coming.
We need to reserve you a slot and we may have other special instructions.
